Lifting parts onto sequence rack
What happened
while loading a part onto a sequencing rack the team member lost control of the part and went down with it and hit their eye on the rack. Team member did not have safety glasses on. Employee did not receive restrictions. Temp Agency required a drug screen and employee acknowledged this but did not take drug screen as instructed and was released by temp agency for failure to follow injury protocol. ([REDACTED])
