Before the incident
On [REDACTED] at approximately [REDACTED]. Injured Employee (IE) ask Employee two ([REDACTED]) to help him move a 2 08 X 8 [REDACTED] prime white solid door from the repair table to the pallet it belonged with on the conveyor. They picked the door up from the trim saw side of the repair table and proceeded to walk between the repair table and door flipper workstation. IE was carrying one end of the door walking backwards. [REDACTED] was caring the other end walking forward. When IE turned the corner around the repair table his finger was mashed between the door that was being carried and a pallet of doors on the conveyor.
