Before the incident
Employee was attempting to make adjustment on the 2nd Pass of the [REDACTED] machine. Emp was trying to loosen a cap head bolt that was under the saw using an Alan Wrench. The wrench broke. When the tool broke it caused employee to lose grip and strike metal of machine. The impact caused a laceration to the base of the employees right thumb on the palm side. Employee sought immediate medical attention and went to the local health clinic.
