EE stated that this was a personal-related ailment that EE has been dealing with for several years. EE described pain in left arm and mentioned carpal tunnel. EE spoke with [REDACTED] again on [REDACTED] and stated that EE 's doctor would not provide a release because EE 's Doctor did not know when EE would be fully release. EE stated that EE 's doctor instructed EE to get covered through workers ' comp. EE was instructed to visit WQ 's clinic doctor for evaluation only.
