Before the incident
While having a conversation with the involved Coworker [REDACTED] about the heat they said something that stood out about not wanting to go to the hospital again for heat illness. Safety Manager asked when they previously went to the hospital and they informed me that they went on [REDACTED] after their shift was over. Safety Manager [REDACTED] advised the Coworker to call axiom to let them know and document the symptoms of the illness and to provide any follow up. Coworker said no and that they were good to go shortly after the visit. Safety Manager had the Coworker fill out a statement form on what happened along with a refusal of medical form. Will know more when I get the employees ER paperwork.
