On [REDACTED] [REDACTED] Employee was working overtime as waterspider in the LayUp department when she was crossing between two roller conveyors and her pants got caught by the corner of one of them causing her to trip and fall facefirst on the floor Employee was immediately assisted by other employees in the area and given first aid Manager asked her if she wanted to go the Clinic but employee refused and preferred to go home as her shift was coming to the end On [REDACTED] [REDACTED] when she returned to work she was with a big bruise on her face and a chipped tooth Manager brought her to the office and called [REDACTED] referred her to the hospital to be checked Employee was released from ER with care recommendations for the bruise but was referred to a dentist for the chipped tooth Employee was sent to the WC Clinic to continue treatment as needed
