Before the incident
It was the end of the shift and the EE was walking to the time clocks to clock out and leave The EE realized he had not locked the man door in receiving and set his bag down turned and started running towards receiving The EE did not look up took 2 steps and ran into a glass vacuum hoist that was positioned next to the aisle The EEs head impacted the piece f equipment and knocked the EE to the floor The door line supervisor was still at his desk and noticed the EE was laying on the floor and went to him and gave him assistance to sit up The EE did not loose consciousness but was dazed for a few minutes The EE said he felt fine after that and did not want to seek medical treatment The EE did not drive himself home as he had a ride After getting up later in the day the employee was experiencing some discomfort where his face impacted the hoist and decided to seek medical treatment The EE was diagnosed with a contusion to the face (cheek) concussion without loss of consciousness and whip
