Before the incident
EE reported he was punching a fab hole into a vinyl bar As he actuated the punch he noticed some vinyl debris on the top of the cylinder as it descended to punch the bar He grabbed the vinyl chads with his left hand and the punch came up pinching his clenched fist between the top of the cylinder and the supporting cross member of the punch above A contusion was noted on his left index and middle fingers The EE had full mobility of his hand and declined treatment on [REDACTED] [REDACTED] Update: EE complained of some remaining swelling and a dull pain in his middle finger when making a fist and would like to get checked EE sent to Concentra for evaluation and prescribed restricted duty
