Before the incident
PIT
What happened
There were two standup PIT (Powered Industrial Truck) operators driving in two different directions. [REDACTED] was coming from the [REDACTED] section and the other operator EE was coming from the dry section. During this time EE had come to a complete stop waiting for the high-speed door to open. Meanwhile [REDACTED] on the [REDACTED] side noticed the high-speed door opening and proceeded at normal speed. Upon approaching [REDACTED] realized that another PIT operator was on the other side. [REDACTED] tried to stop the PIT but due to the momentum his left foot came out of the operator 's compartment and was smashed between both PITs. EE noticed [REDACTED] approaching quickly and ensured he kept his body parts inside his PIT.
