What happened

At [REDACTED] on [REDACTED] two employees while working on line 8 noticed a spillage of line lube on the floor under the bottle conveyor. One of the employees proceeded to the maintenance shop while the other fetched supplies to clean up the residual line lube from the floor. Upon returning from the maintenance shop the employee who went to fetch supplies observed a preform conveyor jam. They ascended mobile stairs to address the issue and unclog the conveyor. As they descended the stairs they were unaware of residual line lube on the bottom of their shoes causing their feet to slip resulting in a fall. The employee utilizing three points of contact managed to prevent a complete fall but sustained an injury to their left arm. The fall resulted in the left arm being ripped above the head and their shoulder took the majority of the impact.
