Employee was lifting a parcel.

General Expediter states she was picking up an oversized box from the floor to place in blue hamper. As placing it into the hamper the hamper rolled out from underneath the box. She came to my desk on [REDACTED] [REDACTED] and stated that she hurt her back on [REDACTED] [REDACTED] while lifting a box. The employee did not state to me where the incident took place and she never submitted a statement regarding the incident. I was later informed through her OWCP claim of where the incident took place.
