What happened
Carrier was bending down to pick up box off the ground and had a sharp pain go from her left side of her back and down her leg. On [REDACTED] at [REDACTED]. carrier was loading her postal truck ([REDACTED]). She picked up a box and placed it in the truck. She turned around to pick up another box off of the ground. As she was bending down to pick up the box she had a sharp pain from the left side of her back go down her leg. Another employee walked by and asked if she was okay. By then she was able to stand up. When she went to walk off the pain came back. At this time she walked back inside and informed management. Upon [REDACTED] she came into the office after her route in pain. At this time she asked me for any paperwork to be able to go to the physicians to get checked out. At this time I had her write a statement and provided the proper paperwork and authorization for her to be examined. I then reach out today on [REDACTED] to supervisor to ensure I properly have this incident repo
