Engaging lever to tilt hopper.
What happened
Employee was in the process of emptying a hopper in an outdoor roll-off container. The employee parked with the hopper elevated above the roll-off exited the forklift to lift the lever to tilt the hopper forward into the roll-off. The employee used his right hand to push the lever which let the hopper tilt forward to dump. The employee 's left hand was placed on the right fork of the forklift and was caught between the fork and the bottom framework of the hopper. The employee pulled his left hand out from between the fork and the hopper framework but the glove was still caught. Employee walked through the [REDACTED] to the breakroom where a co-worker called the Supervisor.

The employee was transported to the emergency department where they were diagnosed with a partial amputation of left index and middle fingertips. The employee was treated and released the same night.
