Before the incident
Employee was transferring a die from the die rack in preparation to install into the press. This process was accomplished using a fork truck.
What happened
Employee removed the die from the rack and lowered the forks on the lift to about six inches off the ground. At this time the employee noticed that the die had shifted on the forks. In an effort to adjust the die the employee dismounted the fork truck and attempted to straighten the die by hand. Once the die began to move the pins on the bottom of the die moved off the side of the fork and pinched the employee 's left index finger between the fork and the pin.
