Before the incident
Operator was operating a mechanical power press punching out blanks for the covers on a filter assembly.
What happened
Operator inserted metal into machine and cycled the machine for the first hit. Upon completion of the first hit the material began to fall out of the machine and the operator instinctively reached to stop it. When he reached for the material his right hand went through the hole in the material and the sharp inner edge cut his are above the wrist on the outer side.
