Before the incident
At approximately [REDACTED] IP was working in assembly area with co-worker (EE1). IP was adjusting the shim as it was out of the adaptor.
What happened
As IP was setting the shim to the correct position of the adaptor EE1 activated the lever for the shim move. This caused the IPs right hand to become pinched between the shim and tube.
