Before the incident
Employee was assisting with the stripping and set up of a 32 ' x 9 ' hyspan. Employee was using a four legged sling and a telehandler was being used to move the form sections. The injured employee was hooking up the rigging.
What happened
While attempting to lift the bulkhead section of the hyspan form the rigging came unhooked from the bulkhead causing the bulkhead to fall to the ground. The rigging was not hooked up properly witch was the contributing factor of the chains coming unhooked. The hooks were attached to the edge of the form rather than to the installed lift points.

Crush injury to left foot ending in a fracture.
