Before the incident
On [REDACTED] at approx [REDACTED] a warehouse technician with approx 5 months of service (DOH6624) was operating hilift [REDACTED] (3rd wk of training) in putaway function at location HH0327 when they sustained a laceration on their right leg The EE had just completed putting away 2 pallets into the second level of pushback rack and was pulling out of the location As they were pulling out of the location the EE was turning within the equipment compartment at the same time They had not retracted the extended forks prior to pulling out which put them closer to the racks behind them in order to pull out(UM found the pallets tilted in the rack) The EE stated that they felt a pinch on their left leg foot against the rack then released the deadman and fell out of the compartment They did not remember what happened next but do know they landed on the floor Based on location we are assuming that a sharp edge of the pushback rack (caused by repetitive rub hitting from hilifts) lacerated the EE
