Before the incident
On [REDACTED] at approximately [REDACTED] an employee with over 17 years of service (DOH [REDACTED] 25 [REDACTED]) was in the picking function on order picker 147 The EE had just made a pick EI0702 and was getting ready to move to their next location As the EE pushed the throttle forward the order picker jerked (sudden stop and go motion) and knocked the EE off balance This resulted in the EE losing their balance and falling backwards over their small pallet of AP The EE landed on their right elbow causing their shoulder to be pushed up The EE had a small rash on their elbow with some swelling shoulder pain and little movement in their arm Axiom was called and both parties initially agreed to the first aid measures On [REDACTED] [REDACTED] EE was seen by HSHS occ health provider MRI was prescribed At [REDACTED] aptMRI results showed 2 torn tendons in the rotator cuff Restrictions were prescribed EE is being referred to orthopedic
