Before the incident
On [REDACTED] the Midwest Fleet experienced a Recordable Injury A driver (DOH 10 2 15) and a store associate from SW Store 703973 in [REDACTED] were unloading the drivers electric pallet jack from the trailer The driver was attempting to steady the jack on the forklift with his hands The driver looked away to see if the forklift had cleared the end of the trailer and could begin to be lowered As he did the pallet jack shifted and his hand was pinched in between the forks of the pallet jack and those of the forklift This resulted in a swelling of the hand that did not allow the driver to make a fist Axiom was notified and first aid measures were followed When improvement was not noted Axiom advised that the driver be seen by an occ health provider That provider sent the driver for an xray which revealed a broken left index finger The driver was fitted with a splint and remains off work until followup visits are made
