Before the incident
The employee was working as the pan-o-mat operator to get the line setup to run. One of the tasks during setup is to adjust the head rollers using the guide wheel based on the product that is being ran. The employee states that as she was turning the guide wheel she opened up the gear box to see if it was opening while she noticed that there was a dough ball in the way. The employee proceeded to remove the dough ball from the chain and sprocket without stopping the machine and got her pinky ring and middle finger caught in the chain. This led to a partial amputation of the tip of her pinky as well as a fracture and stitches due to the lacerations on her middle and ring finger.
