On [REDACTED] at approximately [REDACTED] in [REDACTED] receiving dock contingent worker was breaking down a crate While knocking the lid to the ground contingent worker stepped over it and accidentally stepped on a nail that was still attached to the lid The nail penetrated through contingent workers steel toe shoes and into right foot Contingent worker immediately notified supervisor and the staffing representative The response and firstaid team (ERT) were informed of the incident and promptly arrived The RFT assessed contingent workers condition and provided him with a referral to a clinic for further medical attention
