Before the incident
An EW was receiving crates on the receiving floor between dock door 1011 At the same time an RFT using an 8ft walkie pulled forward to grab a skid to the EWs direct right hand side and one lane over As the RFT backed up the skid slid off the forks bumped a small crate and shifted a bit to the left The EW took a step back The RFT tried a second time and the skid slipped off again The RFT went in for a third time and as he was backing up the skids corner hit two small crates dragging them towards the EW The EW was not able to move in time and the small crate went over their left foot The EW came to me [REDACTED] to report the incident After gathering the information the EW requested ERT and they were called at [REDACTED] Due to the type of injury EMS was also called EMS assessed the injury and deduced that they did not need to be transported to a medical facility The EW was released to go home as their shift had ended They were informed to reach out to [REDACTED] and to reach out with up
