The employee was cleaning the receiving pit where potatoes get transferred. All lock-out tag out was followed and confined space permits where accounted for. The employee started to remove debris.

As the employee was removing debris he stepped in some mud where an anil was located and felt a sharp pain in his foot. Got out of the pit and was taken to the Dr office.

Nail in his foot. Possible exposure to tetanus.

Unable to see poor lighting and debris in the mud.

Stepped in a nail punctured his boot and went through his skin
