Before the incident

The employee was driving a forklift taking product (food) to and from the warehouse out the line location to the add-back system to be added back into our process.

What happened

As the Employee was transporting food back and forth between the line location and the warehouse. The employee was driving down the ramp going backwards carrying a load. The employee had his left foot out of the forklift while traveling backwards. As he was traveling backwards and took the corner to tight and struck a Ballard and pinched caught his foot between the forklift and the Ballard. The employee was wearing safety toes (Composite) required for the facility. The employee was wearing all required PPE for the facility such as hearing protection safety glasses a Bump cap with a suspension system and composite safety shoes. This incident occurred on [REDACTED].

Injury or illness

The injury to the employee was to the left foot digits resulting in several broken toes on the left foot. The employee was taken to ERMIC (local Hospital) For evaluation on 4 1 24. The employee didn�t stay overnight and was sent home for evaluation and medical follow-up. He continued treatment and was just determined 5 3 24 to have the toe amputated.
