Before the incident
The blue packaging box conveyor belt came out of the side guide rail.
What happened
Employee was pushing on a blue smooth top conveyor belt that had come out of the white UHMW side guide rail. EE was wearing white cotton gloves. While employee was pushing belt down another employee started the belt. The employees had went between the belt and a metal guide rail causing a laceration to left pinky and left ring finger (4th and 5th digits). Guide rail was found to have sharp edges.
