Before the incident
V Cooler suffered a major crash [REDACTED] every shift was working to remove damaged cans. Employee on 3rd shift was standing at ground level at the V cooler exit removing damaged cans. The discharge twist was removed from the cooler and the EE was using the jog pendant to rotate the cooler to expose damaged cans then remove them by hand through the discharge opening. Cans were damaged enough to require pry bars to bend and manipulate them out. There was a Production Supervisor on duty but not a supervisor specific to this area in Cell 3 Thermal Processing.
What happened
On [REDACTED] at [REDACTED] Employee was using his hands to remove damaged cans from V cooler through the discharge opening. Tomato paste buildup from these cans limited visibility and made the material slippery. A crushed can got stuck as the Employee pulled cutting through two pairs of gloves and resulting in a laceration to his right thumb.
