What happened

[REDACTED] injured operator indicated that him and another colleague [REDACTED] were working on a set of 3W plates pieces for [REDACTED] came in to request for [REDACTED] sheet bars to be worked on They finished the set they were working on went on break and came back with the intention on working on the [REDACTED] sheet bars [REDACTED] indicated that after break [REDACTED] was grabbing some coffee so he proceeded to the Meco work center He decided to start loading the piece weighing about 1102 Ibs on the Meco rollers to get ready for [REDACTED] to start rolling when he came back [REDACTED] picked up the lifter picked up the sheet bar and started guiding it to the Meco but he indicated that typically he has his right hand holding the lifter and uses the other to guide the material unto the rollers As he moved the piece closer to the Meco rollers he had rested his left hand on the edge of the Meco and just as he was about to lift that hand to hold the piece the sheet bar fell and crushe
