Equipment AccurshearSubstances Involved Titanium plate Operator [REDACTED] was fabricating a safety device for the Intermediate Mill Material involved was titanium [REDACTED] needed to cut the titanium plate and asked coworker [REDACTED] if he was familiar with Accurshear and if he had used in the past [REDACTED] replied to [REDACTED] with yes I have used that shear before to cut up scrap [REDACTED] and [REDACTED] proceeded to place the titanium plate on the bed of the shear [REDACTED] stood back while [REDACTED] used the two hand controls to shear the titanium plate The titanium plate was sheared and [REDACTED] walked toward the bed of the shear to pick up the plate [REDACTED] placed his left hand under the left side of the titanium plate to pick it up not knowing that the end of the plate in the shear was stuck between the blade and the bed of the shear As the blade was coming up and with [REDACTED] left middle finger under the titanium plate to pick it up and move it to a cart it is
