Tightening Safety Valve

TM was notified the pressure relief safety valve from chip tank [REDACTED] was potentially releasing to atmosphere in the cellar. The TM intended to examine the valve to determine if the safety was not tightened sufficiently allowing CO2 to vent from the tank. The TM ascending the ladder to access the top of the tank and confirmed the valve was not seated securely. The TM proceeded to hand tighten the valve. While rotating the valve for the second revolution the breaker bar on the valve blew startling the TM causing him to lean back on the ladder. After the initial release of CO2 from the tank chip beer released out from the safety valve ricocheting off the ceiling back toward the TM on the ladder causing him to lose his grip and veer off the right side of the extension ladder to the ground injuring his left arm. Emergency response services were called and the TM was transported to the local hospital for further evaluation.
