Moving pallets from one location to another

EE was moving pallets on a rider jack [REDACTED] at [REDACTED] by the supervisors desk. EE began to exit the rider [REDACTED] with his left foot. On his descent he accidentally stepped on the corner of a single pallet on the ground he did not see causing him to lose his balance. EE fell backward with his hand still on the throttle of the jack causing the jack to jump forward pinning EE 's ankle between the jack and the pallet. Witness 1 reached for the throttle of the jack and dislodged EE 's ankle freeing EE to get up. EE was okay only some bruising occurred initially. However there were some unintended consequences due to EE 's medication that caused excessive bruising and bleeding under the skin causing some concern about infection.
