What happened
[REDACTED] went into patient room introduced self scanned bracelet. She put tourniquet on patient. Patient ripped off tourniquet hit her in the arm. As she was trying to get needle off of bed patient kicked her in the back and tried to hit her again. [REDACTED] came to nurses station and notified myself. I advised her to go to ED. She was seen by ED physician and reported back to me.
