What happened
Visited the Same Day Clinic for years of ongoing tendonitis carpal tunnel symptoms to work towards getting a PT referral. They encouraged me to submit an LandI claim even though I was hesitant to. Also the date time and location of this S2S is unknown. I also can 't say this happened at work. PH workers comp said to just use the location of the clinic I was seen at.
