What happened
The employee was detaching the pit coupler from the pit when fuel sprayed upwards into her eyes. The agent wears prescription glasses and therefore was not wearing the standard safety glasses. Fuel agent failed to notify management that she required this accommodation in order to wear required PPE whilst fueling is being conducted.
