Before the incident
According to the teammate while removing print plates on the converting machine (6 color) a tab holding the print plate was stuck Using a 1 long paint stir stick she put continuous pressure on the tab As the tab loosened and broke free the momentum of the pressure she was exerting against the tab caused her hands to lunge forward causing her left hand to strike the metal control box that was just above her on the machine The result was a laceration to knuckle of the index and middle fingers
