Before the incident
The injured maintenance teammate was walking toward the display control panel at the Ward loadformer to perform testing during a scheduled [REDACTED] (preventative maint) shutdown Prior to his arrival an assistant at the loadformer had removed the floor plate cover that accesses the loadformer pit (right below the display panel) and was inside the pit performing a [REDACTED] cleanup The maintenance teammate did not see that the floor plate cover was removed and walked over the top of it and fell 5 feet into the opening of the pit The result was a fractured tibia of the left leg
