[REDACTED] placed the guard post into the roll core first aligned the forklift 's fork into the rolls ' core with the guard post. Fork was not centered to the guard post which caused to pushed out the guard post from the core. [REDACTED] tried to align the guard post by pushing it in by hand while pushing it in he got his thumb caught between the guard post plate and the roll.
