Before the incident
Scrapping Tile
What happened
TM had just started shift working on dry rectify line 6 and was scrapping 8x48 tile. When picking up the tile from the line the teammate was using a tossing like motion to move the tile into the hopper. After tossing a piece of tile into the hopper one of the pieces broke and deflected back towards the teammate 's face creating a laceration on the forehead and left eyebrow. At the time of the incident the TM was not wearing his safety glasses.
