What happened
Two maint EEs were called to the [REDACTED] for the peeler table on the payoff end for not working properly. They found a broken Hyd hose and clevis pin under the peeler table. [REDACTED] completed LOTO and replaced the Hyd hose but couldn 't replace the clevis pin due to cylinder alignment. The two maint EEs removed the Hyd lines and tried to manually move the cylinder to align it with the clevis pin. Unable to move the cylinder manually they reconnected the Hyd lines and re-energized the peeler table to align the cylinder. They found the cylinder was bent. [REDACTED] [REDACTED] performed LOTO on the Hyd lines connected to the peeler table cylinder. The injured maint EE was unaware of the 2nd LOTO. While under the peeler table the injured EE disconnected one Hyd line and handed the tooling to [REDACTED] to disconnect the second line. Before [REDACTED] could disconnect the 2nd line the injured EE placed his finger between the alignment holes. The bent cylinder shifted resulting in the
