Before the incident
On [REDACTED] EE was picking boxes from location [REDACTED]. EE extended her arms to reach for boxes in the back of the pallet. EE stated that she picked total of 5 boxes using the same motion for all 5 boxes with her arms extended. After completing her picks EE went to the supervisors office to report a sharp pain in her right shoulder. A member of ERT was called to the office for an evaluation. EE was instructed to contact OHN and assigned to light duty cleaning for the remainder of shift.
