Temporary employee was assembling part [REDACTED] (choo choo train) at an assembly press. Root cause #1 the [REDACTED] had head in line of fire and pulled assembly press arm down. Root cause #2 the [REDACTED] pressed a part removed part and let their hand off the press handle which caused the ram to fall a greater distance meeting the fixture. [REDACTED] did not use lock pin for ram if operations were paused. If you remove the part and do not lock the ram pin the ram will bottom out to the bottom of the fixture and swing arm with force.
