Before the incident
EE arrived at customer location Skyview in [REDACTED]-on-[REDACTED] & opened and secured both his barn doors and [REDACTED]. EE then used the [REDACTED] to gain access into the box of his truck and then began using his E-jack to move a loaded pallet onto his trucks ' [REDACTED]. As EE was moving the E-jack in reverse from the passenger side of the box the E-jack malfunctioned and kept moving in reverse after EE had stopped pressing the reverse button on the E-jack handle. This led the E-jack and pallet to strike EE in his leg and cause him to fall down onto the street from the [REDACTED]. EE then fell onto the ground and struck his right elbow shoulder upper back and chest on the street. EE remained on the ground on his left side after having twisted and turned a few times after the initial fall. Within a few minutes Nursing staff arrived at the receiving area and began to render aid while another employee called for EMS to arrive on scene. EE was eventually given a pillow and some ice
What happened
EE was using his E-jack to remove a pallet of product from the box of his truck onto the lift gate. While doing so EE states that his E-jack malfunctioned and kept moving in reverse in doing so the pallet and E-jack struck EE in his left leg and forced him to fall from the liftgate onto the floor by the rear driver 's side of the liftgate. EE struck his right elbow Shoulder Upper Back & Chest when he fell onto the street. EE remained on the floor on his side and was attended to by nursing staff at Skyview Rehab & also by responding EMS that removed EE from the scene to the local hospital for treatment.
