walking around the truck

What happened

A driver was delivering glass at a customer location on [REDACTED] at approximately [REDACTED]. Before unloading the glass the driver performed the site assessment and did a walk around of the truck. The driver and driver helper then unloaded the glass safely and when the driver went to get into the truck they slipped on some black ice falling on their left side and scraping their right hand on the asphalt. The driver reported the incident immediately and was seen by Bayside Employee Health Center on the same day. The employee was diagnosed with a contusion of the left shoulder and rib cage area which caused pain in their right arm and making it difficult to breathe. The employee was given light duty restrictions that would not allow them to perform any of their work tasks. The employee has a follow up appointment on [REDACTED]
