Worker was preparing to shift a stairway to a different location. The stairway had (2) scaffold board loosely placed on the upper framing of the stairway. [REDACTED] attempted to remove the scaffold boards prior to calling an operator for the lift to prevent the boards from falling during the shifting of the stairway. For reference the stairway measures 3 W x 17 H x 10 L and was fabricated using 4 angle frames along with 10 channels for the diagonal stairway frame. The scaffold boards were laying on the second set of angles framing approximately 9 from the ground. Worker climbed the first level of the angle framing approximately 3 off the ground to reach the boards.

Worker attempted to shift the boards onto the forklifts forks which were level with the boards. Worker had his arms fully extended but was unable to fully grab the boards with enough strength to shift slide them over to the forks. Worker then stood on a horizontal angle framing slightly above to get a better reach. Worker grabbed on to a piece of flat bar that was tack welded to the stairway for support as he attempted to shift the boards over. Worker didnt realize that the flat bar he grabbed was not part of the stairway fabrication member but rather a piece of steel leftover from a welder that used it to hang his welding lead from a previous job. As worker grabbed the flat bar with his left hand he leaned back and used his right hand to push the board closer to the forklifts forks. By doing so the tack weld on the flat bar broke and caused worker to free fall approximately 4. Worker fell on the lower horizontal bracing striking his shoulder against the angle before landin
