Before the incident
Employee finished filling out his inspection paperwork and dropped his rag. He sat down in his chair once he was done and noticed that he never picked up his rag and went to pick it up while sitting in the chair.
What happened
Employee was sitting in an office chair and noticed he dropped a rag on the ground. Rather than standing up and picking it up he leaned over to his right hand side to pick up the rag and due to the counterbalance of the employee and the chair the chair tipped over causing the employee to fall with it hitting his back on the edge of the machine
