What happened
Employee & co worker stood on the grade beam reinforcing steel. holding a section of beam side form while other co workers replaced damaged carton void forms. Employee asked co worker to lay the beam side down so the could go replace other damaged void forms in another area. When employee went to get a new carton void form he walked on the grade beam reinforcing steel then stepped on some unsecured beam side that they had laid down and it tipped over causing employee to fall and strike the left side of his jaw on another beam side that was installed
