Before the incident
Employee was manually pushing a loaded brake shoe cart into the [REDACTED] dump hopper. There is a lip on the blaster dumper employees must overcome when manually loading.
What happened
Operator was pushing a cart of brake shoes into the [REDACTED]. As they hit the lip of the dumper the cart required more force causing IP 's foot to slip and they fell onto a brake shoe in the cart with their chin. An underlying condition existed but not noticed until a later date. IP also had a wrist sprain strain that was not noticed. This ended up becoming a recordable.
