What happened
On [REDACTED] at [REDACTED] the employee was responding to a maintenance request on [REDACTED] manipulator which discharges the 29K. The employee was securing the boom arm of the manipulator with a come-along chain strap to take tension off of the boom making the cylinder that needed repair accessible to manipulate. While accessing the cylinder the come-along and arm reacted to the adjustment of the cylinder causing the arm to shift out and closing the gap between the top and bottom halves of the arm. This closure crushed the employee’s right forearm in the space leading to multiple fractures.
