Before the incident
The technician was repairing wiring for a blue light on a customer 's unit. He chose to splice it into the existing strobe light wiring on the overhead guard. While seated in the forklift he held the sheath with his left hand and cut it left to right using a utility knife in his right. Near the end of the cut increased resistance led him to apply more force.
What happened
The knife slipped through the sheath and cut the inside tip of his left index finger.
