What happened

At [REDACTED] [REDACTED] Maintenance Tech ([REDACTED]) #43730 was assisting and was supposed to be manning the forklift while [REDACTED] Miracle on line 103 removed a hydraulic injection valve. Line was shut down following procedures and LOTO. [REDACTED] [REDACTED] was removing screws and screws were getting jammed which he conveyed to [REDACTED]. It was at that time that the valve burst under pressure near [REDACTED] pushing [REDACTED] back. He fell causing scraps to both arms and a contusion to left knee. It is important to note that [REDACTED] was not wearing the proper Maintenance uniform which requires long sleeve. Had he been doing so the injury may have been prevented or less severe. The team did an excellent job on following safety emergency protocol and called [REDACTED]. The employee did not want to go to the hospital but the paramedics indicated it was necessary to prevent infection because they thought he had a first degree burn (was a friction burn).
